Diabetes and body weight run through the same metabolic machinery, which is why treating them separately so often stalls. Insulin resistance, appetite regulation, sleep, blood pressure and cholesterol move together. A plan that addresses one while ignoring the rest tends to hold for a few months and then drift.

Management is an ongoing relationship. Medication is adjusted as circumstances change, weight is approached as a clinical matter with medical supervision, and the screening that protects long term health, kidneys, eyes, nerves and feet, is scheduled rather than left to chance.

Persistent fatigue, unexplained weight change, temperature intolerance, hair and skin changes, disrupted periods, low mood. Each of these has a long list of explanations, and a hormonal one is somewhere on most of those lists. That is what makes endocrine problems easy to miss and easy to over-diagnose in equal measure.

Most joint pain is mechanical and settles. Some of it is inflammatory, and that distinction matters enormously. Inflammatory disease responds to treatment that does nothing for wear and tear, and it causes damage while it goes unrecognised. Morning stiffness that lasts, swelling, pain that improves with movement, and involvement of several joints at once are the details that shift the picture.

Reflux, bloating, altered bowel habit and abdominal discomfort are among the most frequent reasons adults see a physician, and the overwhelming majority have a manageable explanation. A smaller number do not. Distinguishing between them relies on which symptoms are present, how long they have been there, and whether any of the features that warrant prompt investigation are among them: bleeding, difficulty swallowing, unintended weight loss, anaemia, or a change in bowel habit that persists.

Upper GI Endoscopy

A direct look at the oesophagus, stomach and the first part of the small intestine.

A thin, flexible tube with a camera is passed through the mouth to examine the upper digestive tract. It shows what a scan cannot, the surface of the lining itself, inflammation, ulceration and narrowing, and it allows small tissue samples to be taken.

What to expect

  • Nothing to eat for several hours beforehand
  • Sedation or a throat spray is generally offered
  • Usually a day case procedure
  • If sedation is used, arrange for someone to accompany you home

Colonoscopy

An examination of the large bowel, from the inside.

A flexible camera examines the full length of the large bowel. It is the most reliable way to investigate a persistent change in bowel habit, bleeding, or unexplained anaemia. Polyps can generally be removed during the same procedure, which is why colonoscopy is both an investigation and, often, a treatment.

What to expect

  • Bowel preparation beforehand, following clinic instructions
  • Dietary adjustments for a day or two before
  • Sedation is generally offered
  • Arrange for someone to accompany you home

Sleep Studies

Measuring what happens to breathing during sleep, to explain what happens during the day.

Loud snoring, witnessed pauses in breathing, waking unrefreshed and daytime sleepiness are the usual reasons for a sleep study. Untreated sleep apnoea is associated with raised blood pressure, poorer glucose control and cardiovascular risk, which is why it sits alongside metabolic care rather than separately from it.

What to expect

  • Breathing, oxygen, heart rate and movement recorded overnight
  • May be done at home or in a monitored setting
  • Worn during normal sleep, nothing is invasive

    Before the appointment

    Bring what you already have: previous results, current medications with doses, letters from other doctors. A long standing problem is usually easier to read from its history than from a single day's numbers.

    The consultation

    Most of it is listening. What the symptom feels like, when it started, what makes it worse, what has already been tried. Examination and investigations follow from that conversation rather than replacing it.

  3. 03

    Afterwards

    You leave knowing what is being considered, what is being ruled out, and what happens next, including when to come back and what would be a reason to come back sooner.
