Directing a department is largely about consistency: the same standard of assessment, escalation and follow-up regardless of who is on duty. Patients feel that as care that does not depend on which day they came in.

Internal medicine is the specialty of the whole adult rather than any single organ. It exists for the problems that do not sort neatly into one department: symptoms crossing several systems, long term conditions interacting with each other, or an illness that has not yet been given a name.
